The information collected in this form, references, forms completed as part of your application process and your Working with Children Check, will be used for the sole person of which it is obtained (to assess your suitability to coach a Wyong Roos Junior Rugby League Team). By signing this agreement you agree to the use of this information as well as any other document presented as part of your coaching application for its intended purpose.
